Metastatic Leiomyosarcoma in a transplanted patient. Case presentation
Abstract
Leiomyosarcoma is an unusual mesenchymal malignancy with smooth muscle differentiation (approximatedly 10% of soft tissues sarcomas) even in solid organ transplant reciprents. We report a 29 year-old male who developed a metastatic leiomyosarcoma six years after renal transplantation, while under immunosupression with cyclosporin, azathioprine and metilprednisone. He was studied for recurrent and diffuse abdominal pain, diarrea and malaise; abdominal ultrasound showed multiple hepatic nodules, and other similar images in paravertebral spaces and lung parenchyma.Laparoscopic biopsy was informed as metastatic leiomyosarcoma. The patient was severily ill, so he received chemotherapy in a palliative schedule (adriamycine) and pain treatment including morphine. Immunosupression was not interrupted. Switch to sirolimus was indicated two years latero After three years, the patient shows clinical improvement and the nodules have frankly diminished in radiological studies.
