Tamaño renal, presión arterial, microalbuminuria y filtración glomerular son aconsejables controlar en el seguimiento de niños nacidos prematuros

Resumen

Introducción: Los niños nacidos prematuros crecen con riesgo de daño renal. Objetivos: Estudiar en prematuros la presión arterial, el filtrado glomerular, la relación microalbuminuria/creatininuria, el tamaño renal para determinar la prevalencia de hipertensión arterial y las alteraciones de otras variables. Asociar el tamaño renal con las variables estudiadas. Material y métodos: Diseño prospectivo, transversal. Población: prematuros y controles nacidos a término. Período: 7/2017 a 7/2018. Inclusión: prematuros ambulatorios con edad gestacional ≤ a 32 semanas, peso al nacer ≤ de 1500 g, ≥ a 1 a ≤ 7 años. Se estudiaron: datos perinatales, presión arterial y función renal al momento del control ecográfico. Se midió ecográficamente en ambos riñones, longitud, diámetro anteroposterior y transversal. Se calcularon longitud relativa, volumen absoluto, volumen relativo y volumen predictivo. Análisis descriptivo e inferencial. Resultados: Se estudiaron 43 prematuros y 21 controles, edad media 4,42 y 3,9 años, respectivamente. Se observó hipertensión sistólica en el 21% y diastólica en el 37%; filtrado glomerular alterado en el 74%; relación microalbuminuria/creatininuria alterada en el 80%. La longitud renal media de prematuros fue inferior a los controles en el 53% y fue inferior a la media de Rosenbaum en el 86%. El volumen renal medio en prematuros fue inferior a controles en el 58% y el volumen predictivo inferior en el 45%. Conclusión: Los niños nacidos prematuros entre 1 a 7 años, presentaron alta frecuencia de hipertensión arterial, filtrado glomerular y relación microalbuminuria/creatininuria patológicos, al igual que las longitudes y volúmenes diferentes a los referentes. Podrían considerarse factores de riesgo para la salud renal del prematuro.
